Submit Button on Site Builder directs to error page regardless

I've been struggling with this for 24 hours now and the issue has not been able to be rectified by the micfo support team.

I have created a very simple Form to Mail using the Survey Form in SiteBuilder, when ever I click the Submit Button, it takes me to the onerror.html page. It doesnt matter whether the form has a required field or not.

Can anyone please help.
